Direct injection of 1,25-dihydroxycholecalciferol (1,25-(OH)2-D3) into the parathyroid gland of patients with hyperparathyroidism refractory to 1,25(OH)2-D3 pulse therapy

In 7 patients suffering from hyperparathyroidism refractory to pulse therapy, 1-microgram/ml of 1,25-(OH)2-vitamin D3 solution, the volume of which was approximately 70 % of the estimated volume of an enlarged parathyroid gland, was injected under ultrasonic guidance, 3 times weekly for 2 weeks. Shortly after injection therapy, the blood serum PTH levels fell from 711 +/-213 pg/ml to 255 +/- 138 pg/ml. During the subsequent 4 weeks, the blood PTH concentration decreased further, possibly due to continued use of oral 1,25-(OH)2D3 pulse therapy. (T. Shinzato)
