Effect of the dialysis membrane on mortality rate in patients with acute renal failure

This seems to be the same data set as published in Lancet last fall. Seventy six ARF patients were randomized to be dialyzed with cuprophane vs. PAN membranes using a prospective design. Patient age and APACHE scores were similar. Mortality rate was substantially higher in the CUP group (66 vs. 37%, p < 0.05) with a higher incidence of lethal sepsis in the CUP group, esp. due to gram negative bacteria, e.g., Enterobacter.

Comment: This study is in line with that of Hakim et al in NEJM, who found a strong trend toward decreased mortality with synthetic membranes in ARF, but is not in agreement with a paper by Schafer in Artif Organs in May of 1995. This remains an area of some controversy. (J. Daugirdas)
