Assessment of myocardial structure of dialysis patients

To evaluate whether uremic myocardium shows a peculiar ultrasonic
pattern, we studied 40 HD patients and 17 non uremic hypertensives with
comparable left ventricular mass and septal hypertrophy. The distribution of
gray levels within a region of interest in the intraventricular septum was
analyzed off-line by an array processor based computer.
Compared with hypertensives, HD patients showed a greater myocardial
echogenecity (p<01) and a reduced homogeneity of distribution of gray
levels (p<01). By stepwise regression analysis, CaxP product, degree of
acidosis, HD age and mean arterial pressure accounted for 55% of the
inhomogeneity. HD patients were divided according to presence (MAC+,
20 patients) or absence (MAC-, 17 patients) of mitral annular calcification.
(MAC + patients
showed a more refractile and less homogeneous echo pattern than MAC- patients
(p<01).
