Minimal change nephrotic syndrome resulting from Ehrlichiosis

Hymes et al present a case of minimal change disease in a 16 year old male who was discovered to have elevated IgG titer to Ehrlichia canis which fell significantly within two weeks. The patient was treated with both doxycycline and daily corticosteroids and went into remission leading the authors to suggest that ehrlichiosis may be an unrecognized cause of minimal change disease.

Comment: A much better argument could have been made for Ehrlichia as a cause of minimal change disease had the patient been treated with doxycycline alone. It will be interesting to see if this report stimulates other case reports of this rickettsial organism associated with glomerular disease. (N. Kevin Krane, M.D., Tulane University)
