Nodular parathyroid hyperplasia is refractory to medical
treatments

In this very large series (n=548) of Japanese patients who underwent
parathyroidectomy were included 63 cases resistant to i.v. pulse
calcitriol. 95% of these cases showed nodular hyperplasia and
detailed studies of parathyroid nodules showed a reduced number of
calcitriol receptors. Echotomography of the parathyroid gland was of
value for the identification of patients refractory to calcitriol
because parathyroid glands with a volume >500 mm3 in 86% of cases
presented nodular hyperplasia (Carmine Zoccali, M.D., Reggio
Calabria, Italy).
