TGF-beta and endothelin-1 expression in IgA glomerulonephritis correlates with the progression of renal damage

Vasoactive substances and growth factors have been implicated in the pathogenesis of renal scarring. Rastaldi et al. have elegantly demonstrated that TGF-b and endothelin-1 are heavily expressed both at the glomerular as well as at the interstitial level in renal biopsies of patients affected by IgAN showing moderate-severe histological lesions. On the basis of their results, a pathogenetic involvement of TGF-b and endothelin-1 in the progression of renal damage has been proposed. (Loreto Gesualdo, M.D., University of Bari, Italy)

I liked your presentation, it was the first case presentation I saw on WWW. I wonder how frequent loin pain is in patients with IGAN, I personally cannot remember a single case. Do you advise measuring 24hour urine calcium and uricacid excretion, which has been found increased in children with hematuria, even in the absence of renal calculi? Thanks, Jos Barendregt
